Access you Summary Care Record / Emergency Care Summary
Your health records, stored in a Summary Care Records (SCR), also known as an Emergency Care Summary in Scotland, contain information about you and the care you receive. At Curaleaf Clinic, we require care records for all new patients along with any clinic letters.
You can telephone your GP practice and request that the medical secretary sends this information via email to [email protected] or access your record via the NHS App if you an English patient.
Once received, we will send you a booking link to make an appointment.
Psychiatry
Patients under the care of one of our Consultant Psychiatrists are required to provide specialist letters, in addition to the Summary Care Record / Emergency Care Summary. Please request these from your GP or specialist and send to [email protected]
